Information To Bring

Helpful checklist to make the most of your visit

  • Photo ID (driver’s license, passport, etc.)
  • Dental insurance card (if applicable)
  • Medical insurance card (sometimes requested for billing)
  • List of current medications (prescriptions, supplements, vitamins, over-the-counter drugs)
  • List of allergies (medications, latex, anesthesia, etc.)
  • Medical history (surgeries, conditions like diabetes, heart disease, etc.)
  • Previous dental records/X-rays (if transferring from another dentist—call ahead to request copies)
  • Emergency contact information

Payment Forms

  • CareCredit© accepted.
  • Most major credit cards accepted. 

Personal Notes & Questions

  • List of dental concerns/symptoms (pain, sensitivity, cosmetic concerns, grinding, etc.)
  • Questions for the dentist (treatment options, costs, prevention tips)
  • Timeline or goals (e.g., “I want whitening before my wedding,” “I’m interested in Invisalign”)

Practical Items

  • Mouthguard/nightguard/retainer (if you wear one)
  • Dental appliances (partials, dentures, aligners, etc.)
  • Lip balm (helpful if you have long treatments—lips can dry out)
  • List of preferred pharmacies (for prescriptions if needed)

Optional but Helpful

  • Water bottle (before/after appointment—most offices prefer no food/drink inside)
  • Headphones/earbuds (for comfort during long procedures)
